You have to meet her in citadel 3 times and always choose the 'paragon' option, first in docking area after palaven, then in spectre office and later in that apartment. I played the DLC using a perfect ME save (no one died on the suicide mission, started the Citadel DLC right before the point of no return having done all of the side quests possible including the Miranda side story.) I never got a prompt to meet Miranda and she never showed up at my party or was an option to invite.
